لا تبدأ الأزمة القلبية دائمًا بالمشهد الدرامي الذي اعتدنا رؤيته في الأفلام؛ فقد لا يمسك الشخص صدره فجأة ثم يسقط أرضًا، وإنما قد تظهر الأزمة في صورة أعراض تبدو عادية، مثل الحموضة أو الإرهاق غير المعتاد أو ضيق التنفس بعد مجهود بسيط، وهو ما قد يدفع البعض إلى تجاهلها وتأخير طلب المساعدة الطبية.
وبحسب موقع Hindustan Times، حذر مدير قسم أمراض القلب التداخلية في مستشفى «ميدانتا» بمدينة نويدا الهندية الدكتور نيشان تياجي من أن الجسم غالبًا ما يرسل إشارات تحذيرية قبل أن تصبح الأزمة القلبية حرجة، مشيرًا إلى أن الخطورة تكمن في تفسير هذه العلامات باعتبارها أعراضًا بسيطة لا تستدعي القلق.
وأوضح الطبيب أن الأزمة القلبية تحدث عندما ينخفض تدفق الدم إلى جزء من عضلة القلب بشكل حاد أو ينقطع تمامًا، وكلما استمر الانسداد لفترة أطول زاد خطر تعرض أنسجة القلب لتلف لا يمكن عكسه.
ألم الصدر ليس الشكل الوحيد
ويُعد الشعور بعدم الراحة في الصدر من أكثر الأعراض المعروفة للأزمة القلبية، لكن الدكتور تياجي أكد أنه لا يظهر بالضرورة في صورة ألم شديد.
وقد يشعر المصاب بدلًا من ذلك بضغط أو ضيق أو ثقل أو امتلاء أو إحساس بالعصر في منتصف الصدر أو عبره، وقد يستمر الشعور عدة دقائق، ثم يخف لفترة قصيرة قبل أن يعود مجددًا بصورة أكثر حدة.
وحذر الطبيب من الخطأ الشائع المتمثل في انتظار اشتداد الألم قبل طلب المساعدة، مؤكدًا أن الشعور حتى بآلام أو انزعاج خفيف وغير معتاد في الصدر يجب أخذه على محمل الجد، خصوصًا إذا ترافق مع أعراض تحذيرية أخرى.
الألم قد ينتقل إلى أماكن أخرى
ولا تقتصر أعراض الأزمة القلبية على منطقة الصدر، إذ يمكن أن ينتشر الشعور بعدم الراحة إلى ذراع واحدة أو الذراعين، والكتفين، والظهر، والرقبة، والفك، أو الجزء العلوي من البطن.
وقد يفسر الشخص ألم الفك أو الشعور المزعج بين لوحي الكتف باعتباره مشكلة عضلية، لكن وجود هذه الأعراض بالتزامن مع ضيق التنفس أو التعرق أو الغثيان أو ألم الصدر يستدعي التعامل معها بجدية وعدم افتراض أنها مجرد عسر هضم أو إجهاد عضلي.
ضيق التنفس علامة لا ينبغي تجاهلها
ويُعد ضيق التنفس المفاجئ أو غير المعتاد من العلامات المهمة التي قد تشير إلى وجود مشكلة قلبية، حتى في غياب ألم الصدر.
وقد يشعر الشخص بأنه غير قادر على أخذ نفس كامل، أو يصاب بضيق تنفس شديد بعد نشاط بسيط، أو يعاني من صعوبة في التنفس حتى أثناء الراحة.
وأكد تياجي أن ضيق التنفس يمكن أن يظهر مع ألم الصدر أو من دونه، ولذلك فإن ظهوره بصورة مفاجئة وغير معتادة يستدعي الانتباه.
فخ الحموضة
من أكثر الأمور التي قد تتسبب في تأخير اكتشاف الأزمة القلبية، الخلط بينها وبين الحموضة أو عسر الهضم.
وأوضح الطبيب أن الحموضة وحرقة المعدة وعسر الهضم أعراض شائعة، وغالبية الحالات لا تكون مرتبطة بالقلب، لكن الأزمة القلبية قد تتشابه معها في بعض الأحيان.
ولهذا، فإن الشعور الجديد أو غير المبرر بحرقان أو ثقل أو انزعاج في الصدر لا ينبغي افتراض أنه مجرد حموضة، خصوصًا إذا صاحبه تعرق أو ضيق تنفس أو غثيان أو دوخة، أو امتد الشعور إلى مناطق أخرى من الجسم.
النساء وكبار السن ومرضى السكري
وأشار تياجي إلى أن بعض الفئات تكون أكثر عرضة لظهور أعراض غير نمطية للأزمة القلبية، ومن بينها النساء وكبار السن والأشخاص المصابون بالسكري.
وبينما يظل ألم أو انزعاج الصدر من الأعراض الشائعة لدى مختلف الفئات قد تعاني النساء بصورة أكبر من أعراض مثل الغثيان والقيء والإرهاق الشديد أو الضغط في أسفل الظهر بدلًا من ألم الصدر التقليدي.
ماذا تفعل عند الاشتباه في أزمة قلبية؟
عند ظهور مجموعة من هذه الأعراض، فإن النصيحة الأساسية هي عدم الانتظار لمعرفة ما إذا كانت ستختفي من تلقاء نفسها.
وقال الدكتور تياجي إن الاشتباه في الإصابة بأزمة قلبية يجب التعامل معه باعتباره حالة طارئة، داعيًا إلى الاتصال بخدمات الطوارئ فورًا، وعدم قيادة السيارة إلى المستشفى بنفسك إذا كانت خدمة النقل الطبي الطارئ متاحة.
وتكمن أهمية سيارة الإسعاف في أن الفرق الطبية يمكنها إجراء الفحوص الأولية، وتقديم الإجراءات اللازمة لتحقيق الاستقرار، وإبلاغ المستشفى بحالة المريض وتجهيز فريق قسطرة القلب قبل وصوله.
وبينما ينتظر المريض وصول المساعدة، ينبغي عليه التوقف عن أي مجهود بدني والجلوس في وضع مريح ومدعوم لتقليل الضغط على القلب.
هل يمكن تناول الأسبرين؟
أوضح الطبيب أن الأسبرين قد يكون مفيدًا عند الاشتباه في الإصابة بأزمة قلبية، لأنه يمكن أن يقلل من تكوّن جلطات الدم، لكنه ليس مناسبًا للجميع، فالأشخاص الذين يعانون حساسية تجاه الأسبرين أو بعض مخاطر النزيف لا ينبغي لهم تناوله.
وتسمح الإرشادات الطبية الحالية للبالغ الواعي الذي يعاني ألمًا في الصدر غير ناتج عن إصابة بمضغ وابتلاع جرعة قياسية من الأسبرين بعد طلب خدمات الطوارئ، بشرط عدم وجود حساسية أو موانع طبية معروفة. وفي حال وجود أي شك، فمن الأفضل انتظار توجيهات المختصين.
إذا فقد المصاب الوعي
إذا أصبح الشخص غير مستجيب وتوقف عن التنفس بصورة طبيعية، فينبغي على الموجودين بجواره البدء فورًا في الإنعاش القلبي الرئوي (CPR) واستخدام جهاز مزيل الرجفان الخارجي الآلي (AED) إذا كان متاحًا.
وشدد الدكتور تياجي على أن أهم رسالة هي أن الأزمة القلبية لا تظهر دائمًا بالشكل الدرامي الذي تعرضه الأفلام، مؤكدًا أن التصرف المبكر عند الاشتباه في وجود أزمة قلبية أكثر أمانًا بكثير من الانتظار حتى تتفاقم الأعراض.
وبالتالي، فإن الشعور غير المعتاد في الصدر، أو ضيق التنفس المفاجئ، أو الألم الممتد إلى الذراع أو الفك أو الظهر، خصوصًا إذا ترافق مع التعرق أو الغثيان أو الدوخة، يستحق تقييمًا طبيًا عاجلًا، بدلًا من افتراض أنه مجرد حموضة أو إرهاق عابر.
A heart attack does not always begin with the dramatic scene we are used to seeing in movies; a person may not suddenly clutch their chest and fall to the ground. Instead, the heart attack may present itself in the form of symptoms that seem ordinary, such as acidity, unusual fatigue, or shortness of breath after minimal exertion, which may lead some to ignore them and delay seeking medical help.
According to Hindustan Times, Dr. Nishan Tyagi, the director of the Interventional Cardiology Department at Medanta Hospital in Noida, India, warned that the body often sends warning signals before a heart attack becomes critical, noting that the danger lies in interpreting these signs as simple symptoms that do not warrant concern.
The doctor explained that a heart attack occurs when blood flow to a part of the heart muscle is sharply reduced or completely cut off, and the longer the blockage lasts, the greater the risk of irreversible damage to the heart tissue.
Chest pain is not the only form
The feeling of discomfort in the chest is one of the most well-known symptoms of a heart attack, but Dr. Tyagi confirmed that it does not necessarily manifest as severe pain.
Instead, the affected person may feel pressure, tightness, heaviness, fullness, or a squeezing sensation in the middle of the chest or across it, and this feeling may last for several minutes, then lessen for a short time before returning more intensely.
The doctor warned against the common mistake of waiting for the pain to intensify before seeking help, emphasizing that even mild and unusual chest pain or discomfort should be taken seriously, especially if accompanied by other warning symptoms.
Pain may radiate to other areas
The symptoms of a heart attack are not limited to the chest area; the feeling of discomfort can spread to one or both arms, shoulders, back, neck, jaw, or the upper part of the abdomen.
A person may interpret jaw pain or discomfort between the shoulder blades as a muscular issue, but the presence of these symptoms alongside shortness of breath, sweating, nausea, or chest pain should be treated seriously and not assumed to be merely indigestion or muscle strain.
Shortness of breath is a sign that should not be ignored
Sudden or unusual shortness of breath is one of the important signs that may indicate a heart problem, even in the absence of chest pain.
A person may feel unable to take a full breath, experience severe shortness of breath after minimal activity, or have difficulty breathing even at rest.
Dr. Tyagi confirmed that shortness of breath can occur with or without chest pain, so its sudden and unusual appearance warrants attention.
The acidity trap
One of the most common issues that may delay the detection of a heart attack is confusing it with acidity or indigestion.
The doctor explained that acidity, heartburn, and indigestion are common symptoms, and the majority of cases are not related to the heart, but a heart attack can sometimes resemble them.
Therefore, a new or unexplained feeling of burning, heaviness, or discomfort in the chest should not be assumed to be merely acidity, especially if accompanied by sweating, shortness of breath, nausea, dizziness, or if the sensation extends to other areas of the body.
Women, the elderly, and diabetics
Dr. Tyagi pointed out that certain groups are more likely to present atypical symptoms of a heart attack, including women, the elderly, and people with diabetes.
While chest pain or discomfort remains common symptoms across various groups, women may experience symptoms such as nausea, vomiting, severe fatigue, or pressure in the lower back instead of the traditional chest pain.
What to do if a heart attack is suspected?
When a combination of these symptoms appears, the primary advice is not to wait to see if they will go away on their own.
Dr. Tyagi stated that suspicion of a heart attack should be treated as an emergency, urging individuals to call emergency services immediately and not to drive themselves to the hospital if emergency medical transport is available.
The importance of an ambulance is that medical teams can conduct preliminary examinations, provide necessary stabilization procedures, and inform the hospital of the patient's condition and prepare the catheterization team before arrival.
While waiting for help to arrive, the patient should stop any physical exertion and sit in a comfortable and supported position to reduce pressure on the heart.
Can aspirin be taken?
The doctor clarified that aspirin may be beneficial when a heart attack is suspected, as it can reduce the formation of blood clots, but it is not suitable for everyone. People who are allergic to aspirin or have certain bleeding risks should not take it.
Current medical guidelines allow a conscious adult experiencing chest pain not caused by injury to chew and swallow a standard dose of aspirin after calling emergency services, provided there are no known allergies or medical contraindications. In case of any doubt, it is best to wait for guidance from specialists.
If the person loses consciousness
If a person becomes unresponsive and stops breathing normally, those nearby should immediately begin cardiopulmonary resuscitation (CPR) and use an automated external defibrillator (AED) if available.
Dr. Tyagi emphasized that the most important message is that a heart attack does not always present in the dramatic way depicted in movies, stressing that early action when suspecting a heart attack is much safer than waiting for symptoms to worsen.
Thus, an unusual feeling in the chest, sudden shortness of breath, or pain extending to the arm, jaw, or back, especially if accompanied by sweating, nausea, or dizziness, deserves urgent medical evaluation rather than assuming it is merely acidity or transient fatigue.
